The most common side effect is darkening of the eyelid (periocular hyperpigmentation). This is usually reversible when you stop taking the medication. Iris darkening occurs primarily in light-colored eyes (green, hazel, blue-green) and is not reversible.
In addition, changes in the fat surrounding the eye can occur, resulting in collapse of the upper eyelid, sunken eyes, and altered eyelid creases. This can permanently alter the appearance.
Furthermore, dry eyes or irritation, burning, redness, and watery eyes may occur.
Although less potent than medicated eye drops, the serum can lower eye pressure and interfere with glaucoma treatment.
Synthetic variants such as isopropyl cloprostenate and ethylcloprostenolamide also have similar effects and risks as bimatoprost. Many of these ingredients are not officially approved for use around the eyes, but are still found in cosmetics.
