What is pubalgia?

Pubalgia, also known as symphysis pubis dysfunction (SPD) or osteitis pubis, is a condition that causes pain in the region of the pubic bone and surrounding structures in the pelvic area.

It is often seen in athletes, especially those who play sports that place a lot of strain on the pelvic and groin area, such as soccer, hockey, and running. It can also occur in pregnant women due to the changes and strain on the pelvic area.

The condition often occurs due to repetitive strain on the muscles, tendons, and joints around the pubic area. This can lead to irritation, inflammation, and sometimes degenerative changes. Uneven muscle strength between the abdominal muscles and the muscles of the thighs and groin can place extra strain on the pubic bone. Sometimes pubalgia can occur after a specific trauma or injury, such as a sudden movement or impact to the pelvic area.

Pubertal pain is the most common symptom, often experienced as a dull ache that may radiate to the groin, lower abdomen, or inner thighs. Activities such as running, jumping, twisting, or sudden changes in movement can make the pain worse.

Treatment for pubalgia involves avoiding activities that aggravate the pain, pain management, and targeted exercises to strengthen and stretch the muscles around the pelvis. In rare, severe cases, surgery may be necessary to repair damaged tissues.
