Is every PRP treatment equally effective?

Not every PRP treatment is the same. The quality and clinical effectiveness of PRP (platelet-rich plasma) depend heavily on several factors, including the device, the method, the composition of the PRP, and the person administering it.

There are dozens of commercial PRP systems (RegenLab, Arthrex Angel, EmCyte, PCCS). These vary greatly in the number of platelets effectively concentrated. Some systems deliver 2x the concentration, others 6–8x. Insufficient concentration results in less effectiveness. Too high a concentration can cause inflammation.

Leukocyte-rich LR-PRP (with white blood cells) is better for tendon injuries but causes more inflammation. Low-leukocyte LP-PRP (low in white blood cells) is better for osteoarthritis or joints and causes less irritation. Not every device can produce both.

Some inexpensive systems deliver variable quality, sometimes with almost no platelets or too many red blood cells.

Not every indication responds equally well to PRP. There is some evidence for knee osteoarthritis (mild to moderate), tennis elbow, patellar tendon problems, Achilles tendinopathy, and hair loss (androgenetic alopecia).

The evidence is more limited for the rotator cuff, facet joints, and plantar fascia. There is no evidence for back pain without a specific diagnosis and general cosmetic applications.
